- Promote Health eldercare services and programmes
- Conduct/support active ageing services suitable to our seniors and encourage active participation from seniors. The services include: conducting CS assessment, organize and deliver active ageing programs, provide befriending services (home visits) and support seniors in monitoring their vital signs.
- Where required, coordinate care and support services through internal referral or external agencies
- Support any seniors outreach events and programmes that may happen after office hours and on weekends when needed
- Carry out administrative functions such as data entry, record keeping and report submissions in a promptly and accurately
- May require to perform additional duties as requested
Job Requirements
- Min A levels; NITEC; Diploma in any course
- Able to commence work within short notice
