We are currently seeking a Senior / Medical Social Worker for both Acute and Community Settings. In this role, your responsibilities will include:
- Conducting assessments of the psycho-social status of patients and their families/caregivers to develop appropriate care plans.
- Providing counseling and support to patients and their families/caregivers dealing with illness.
- Collaborating closely with community partners to ensure the welfare of patients.
- Delivering public education talks on relevant topics.
- Maintaining effective working relationships with other disciplinary teams to create comprehensive patient care plans.
- Coordinating with other service providers to ensure seamless continuity of care.
- Facilitating sessions for students and other professionals to share knowledge and expertise.
- Ensuring accurate documentation for case management and audit purposes.
Requirements:
- Minimum of a Bachelor's Degree in Social Work.
- Passionate about making a difference in the lives of patients and their families.
