Job Responsibilities:
- Interview and assess patients' financial assistance eligibility
- Provide counselling to patients and their families
- Educate patients and families on available community resources
- Document counselling, reports and treatment plans
- Write referrals to agencies for patients in need
- Assist in planning and organizing group counselling and seminars
- Assist in the compilation of statistics requested by MOH
- Lead quality improvement efforts
Requirements:
- Degree in Social Work
- Compassion and care for patients
- Enjoy working in fast pace environment
