1. Assessment (Social Investigation)
- Schedule investigation interviews promptly with the Intake Centre and client.
- Interview the client and relevant persons / stakeholders to make a reasonable and fair assessment of the client.
- Prepare the social investigation report for vetting by the supervisor.
- Right site the client to a community setting or welfare home within the DPA (30-day timeframe).
2. Casework
A. Engagement & Assessment
- Develop professional & engaged relationships with clients and other key service stakeholders (e.g. Family Service Centres (FSCs), Social Service Offices (SSOs), Ministry of Social and Family Development (MSF), etc.)
- Conduct mentoring sessions for all assigned clients as per the service model requirements.
- Conduct needs assessment and risk assessment for complex cases and refer to the relevant agencies for intensive case management.
- Co-manage complex cases with the relevant agencies regularly.
B. Case Review & Documentation
- Prepare social reports / case recordings following the requirements of New Hope CS and ensure documentation is in place.
- Ensure the database is timely updated.
- Consistently collect relevant documents from clients to prepare for submission to the funder at the end of the contract period.
- Meet the deadline for submission of reports.
C. Goal Setting & Intervention Plans
- Work closely with clients and set goals during mentoring sessions. Upon the next session, follow up and review the goals set.
- Participate in case conferences as part of a multi-disciplinary team for complex cases.
D. Supervision
- Attend individual and group supervision as scheduled regularly.
- Meet the supervision hours required by the funder.
3. Group Work
- Conduct support group sessions on a quarterly basis either during weekday evenings and/or or weekends.
- Formulate, plan, and deliver sessions for clients with similar issues.
- Conduct yearly needs analysis to assess the needs of clients.
4. Community Work / Outreach / Events for Clients
- Conduct community outreach to assigned clients at least twice in the contract period as arranged.
- Attend events organised for clients during weekends and/or weekday evenings (e.g. Family Retreats)
- Formulate, plan, and deliver training for clients
- Attend training organized for clients either during weekday evenings or weekends
- Create bi-monthly e-newsletter for clients
5. Secondary Duties: Centralised Triage Team (CTT)
- Attend each referral to New Hope CS and conduct intake assessment with the Referral Agency (RA) for individuals.
- Upload the Intake assessment report into eCMS and highlight it to Team leads for the right siting.
- Staff will need to have access to eCMS and make timely updates.
6. Candidate Requirements
Technical Competencies:
- Degree in Social Work or Post-Graduate Diploma in Social Work from an accredited institution
- Accredited as a Registered Social Worker with SASW
- At least 2 years of Social Work / Case Management experience is preferred.
Core Competencies:
- Strong interpersonal and communication skills
- Understand the principles of engaging and working with vulnerable families and individuals
- Case management and casework knowledge and skills
- Good report-writing skills
- Willing to work with the homeless and destitute
