Your Responsibilities: -
- Conduct home visits for referred chronic patients and perform nursing assessments on patients.
- Collaborate with physicians for medical assessments and follow-up visits.
- Monitor medication usage and adherence as per the physician's instructions.
- Administer nursing procedures, including wound care, catheter insertion, nasogastric tube placement, and tubing maintenance as necessary.
- Identify patients and families requiring financial aid and direct them to the Medical Social Worker/Counselor.
- Recognize patients and families in need of psychological support or counseling and refer them to the Medical Social Worker/Counselor.
You will need: -
- Min 1 year or more relevant nursing experience
- Minimum Diploma in Nursing
- Registered with the Singapore Nursing Board
- Possession of a valid BCLS and AED certification
- Completed Covid-19 vaccination
