Job Scope
A. Programme Development and Implementation
i. Provide care coordination and service linkage to clients with or who are at risk of mental health issues and/or dementia, with the appropriate resources and support including caregiver support.
ii. Followed up with assessment and review information and referral service and monitoring of clients with mental health condition
iii. Plan and conduct outreach to the community and stakeholders to promote mental wellness and mental health awareness, including community mapping
iv. Plan and conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support.
v. Plan and organize structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients.
B. Administration and Staff Development
i. Responsible for maintaining client records that are accurate and timely for submission to AIC for data analysis, in accordance with theCREST programme requirement.
ii. Keep and maintain client and programme data and documents, in accordance with the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.
iii. Review and analyze collated data and monitor Participate in Support in the development of CREST service planning document and manual including set up requirements (infrastructure, manpower, training, recruitment) and processes (outreach, screening, mental health information, service linkage & follow-up).
iv. Participate in relevant meetings with key stakeholders including AIC to share key learning points where required.
Candidate Requirements
Work Area: Anchorvale and Buangkok as well as Boon Lay
· Experience in the health and social services sector, particularly in caring for persons with mental health
· Strong interest in advocating mental wellness
· Possess good interpersonal skills, proactive in outreach
· Highly motivated and service mindset to serve the community
· Able to work independently and a team player
· Abliity to speak dialects and Mandarin to engage seniors who cannot speak English will be a plus point
· Good time management, efficient and able to multi-task
