Job Scope
- Obtains history, gathers triage information, assesses and evaluates patient health concerns made via phone, fax, email or other modes of communications to determine right siting of care and provide health advice according to established guidelines and protocols
- Develops, implements, and evaluates a plan of care for each caller's presenting symptoms
- Collects and analyzes data to establish health priorities and interventions.
- Streamline the triage process and shorten the waiting time for patients requiring Emergent care or Specialist consult.
- Maintains and safeguards confidentiality of patient health information and medical records.
- Communicates complex triage case with the relevant clinician.
- Network with the clinicians from clinical departments to establish and update clinical triage protocols
- Provide daily updates and monthly reports of the Triage services.
- Provide regular in-house training and assist in preparing quiz for the department so as to improve the team's medical knowledge.
Requirements
- Min. Dipoma / Degree in Nursing from a recognised institution
- Valid SNB
- Min. 2-4 years’ of nursing work experience preferably from an acute setting
- Competent in deciphering medical jargons, abbrieviations and triage referral letters competently
