Job Purpose:
You shall be responsible for family/individual counselling, group work, assessment and recommendations for financial assistance and case management. Conduct casework and manage case consultations.
Key Responsibilities:
Individuals and Families
- Assess, plan and provide intervention for students/clients and their families in partnership with other professionals and school/ centre personnel when required.
- Provide progress and termination reports to ensure case management in accordance to the Case Management Review procedures where applicable.
Group Work, Therapy / Training
- Assess and provide training for teachers in working with students and their families.
Financial Assessment and Assistance
- Conduct professional assessment (information gathering, home visits) and recommend / apply for subsidy in consultation with relevant guidelines.
- Contact and refer students/ clients and their families to relevant agencies for assistance as deem appropriate after assessment.
- Work with community agencies in service delivery for students/ clients and their families.
Professional Consultation
- Provide consultation to teachers/ parents of services offered in the Social / Human Services sector. (I.e. VWOs, NCSS etc.).
- Provide consultation to teachers / parents and other professionals on relevant external systems such as social care, healthcare and legal etc. (i.e. general information on hospitals, welfare homes, courts etc.)
- Where appropriate and necessary, to assess the case and make suitable referrals.
Job Requirements:
- Degree and/ or Post Graduate Diploma in Social Work or equivalent from recognised universities.
- Must be registered with Singapore Association of Social Worker (SASW) Accreditation Board.
- Possess minimum 2 years of relevant work experience.
- Possess experience working with persons with mild intellectual disabilities (MID).
- Patient, team player and able to co-create new content tailored for students/trainees with MID.
- Passionate, well organised, able to independently.
- Able to communicate with students, trainees, caregivers and associates of different culture/ backgrounds, preferably conversant in major local dialects (in order to provide consultation to local dialects speaking teachers, parents and students.)
