You will be responsible for the provision of pharmaceutical care by working collaboratively with a multidisciplinary healthcare team to promote wellness, maintain current health, and/or intervene in acute or chronic illness. You will also identify, resolve and prevent drug-related problems.
In addition, you will recommend and monitor patient drug therapy and provides drug information. Your role requires to interact with patients, family, and other health care providers as appropriate to assure continuity of services and to achieve desired patient outcomes through safe, timely, and cost-effective drug therapy. You will need to ensure the accurate and timely distribution of pharmaceuticals, and provides education to patients, Pharmacy staff and other healthcare practitioners.
Job Requirements
- Degree/Master Graduate from a School of Pharmacy recognised by the Singapore Pharmacy Council
- Full registration with the Singapore Pharmacy Council.
- Valid membership with local professional body/association is preferred where applicable.
- Experience as a Pharmacist in relevant clinical settings and demonstrated competency will be an advantage
