JOB DESCRIPTION
- Call and follow up with caregivers whose loved ones were referred to our programme.
- Arrange with carers to bring their loved ones for pre-admission assessment.
- Conduct the pre-admission assessment and 14th session assessment.
- Conduct InterRai assessment.
- Input client’s assessment results, and other information into a system for tracking and submission purposes.
- Track client’s attendance for monthly report.
- Liaise and update different stakeholders such as doctors, AIC, caregivers etc.
- Familarise with DSG FOW mobile application, able to troubleshoot if the App encounters problems.
- Involve in planning and facilitating individual and group sessions.
- Assist in the development and introduction of new activities.
- Assist in the collection and collation of data and statistics and feedback for Manager’s reporting.
- Other duties as assigned.
REQUIREMENTS
- At least GCE ‘A’ Level or Diploma (in eldercare field) and above.
- Passionate to work with elderly and/or persons living with dementia.
- Leadership quality and good team player.
- Creative and hands-on person with strengths in recreational activities such as baking, arts, crafts, music, dance, etc.
- Proficient in Microsoft Office.
- 5-day work week: Monday-Friday.
