Metta Day Rehabilitation Centre for the Elderly (慈光乐龄日间康复中心) was established in June 1998 to provide rehabilitative care to those suffering from physical impairment due to stroke, Parkinson’s disease or other injuries.
With a maximum capacity of 75 from various race, language or religion, our services are geared towards maximising our patients’ independence level, while offering respite to their families and caregivers. Our centre executes our vision of care to maximise their functional level by providing a comprehensive range of rehabilitative and social support services all under one roof.
We focus on assisting our daily clientele in regaining maximum functional capability through rehabilitative treatment and engaging in active lifestyles by means of social programmes.
As a Rehab Supervisor, the candidate is responsible in the following areas:
1. Main Responsibilities
1.1. Manage daily centre operations
· Ensure smooth management of centre operations.
· Assist manager in planning and evaluation of budget and resources utilitsation.
· Ensure that resources are utilised effectively in a manner that is beneficial to the organization.
· Optimise client capacity for safe care delivery.
1.2. Manage person-centred care
· Plan and organize person-centred activities for clients with with various clinical conditions in collaboration with inter-professional teams.
· Perform holistic assessment (bio-psycho-social-emotional),
· Contribute to formulation of individualized care plans and periodic evaluation by working closely with Physiotherapist and Occupational Therapist.
· Manage admission and discharge of cases in and out of Medical Care services.
· Manage transition of care coordination within and between teams and across settings.
· Lead clinical and quality improvement initiatives.
· Review current practices to identify areas for improvement to maintain standard of care.
· Evaluate the care delivered to the clients and providing assistance to reinforce the standard practices in accordance with organization prolicies and guidelines.
· Review existing infection control and prevention practices and ensure that practices meet quality standard of care.
· Promote active engagement and participation of clients
· Proactively engage clients and caregivers through regular updates and communication.
1.3. Supervision and Manpower management
· Enable staff to assume responsibilities for patient care with delegation, support and coaching.
· Manage staff’s productivity and innovation.
· Facitilate positive change management for smooth adoption of new technologies, productivitiy initiatives, processes and procedures
· Maintain an environment conducive for effective staff learning.
· Plan and provide training for staff with necessary skills in carrying out their duties.
· Use manpower resources for optimal patient care outcome through effective scheduling, skills mix and staffing assignments.
· Conduct performance appraisals of staff to support their professional development.
· Promote staff retention and performance outputs.
· Identify talents for further development.
1.4. Provide assistance with managerial tasks
· Act as a deputy to Medical Care Manager in his/her absence.
· Assist in planning and evaluation of budget and resources utilitsation.
· Assist in resolving issues related to care, coordination and service lapses.
· Facilitate resolution of interpersonal and interdeisciplinary conflicts and grievances.
· Assist in incident report, investigation, management and risk management.
· Assist in developing new services, where applicable.
· Assist to prepare reports to various external authorities as well as internal management committees.
· Evaluate effectiveness of services delivered through survey and feedbacks.
1.5. Other Responsibilities
· Upkeep the confidentiality of the beneficiaries and compliant with PDPA.
· Participate in Metta Welfare Association’s fund raising activities.
· Attend continuing education to improve knowledge, competency and standard of service delivery
· Assume additional responsibilities as assigned by supervisors.
1. Requirements
Experience
· Minimum 5 years of relevant working experiences in various healthcare settings
· Minimum one year of supervisory experience
Qualifications
· Degree in Nursing (Minimum) or Eldercare
· Post-graduate or Advance diploma in Eldercare/ Community Care preferred
Other Requirements
· Registered with Singapore Nursing board
· Valid BCLS and AED certification
· Completed Covid-19 vaccination
· Computer and IT skills
