Responsibilities:
- Perform medications reconciliation & review, and ensure the appropriateness of patient drug therapy
- Dispense medications and give appropriate medications counselling to the patients & caregivers
- Provide timely and accurate drug information to healthcare professionals and patients / caregivers
- Involve in the education and training of pharmacy staff and other healthcare professionals
- Participate in projects/activities which support the goals of the department and hospital
Requirements:
- Degree in Pharmacy
- Registered Pharmacist with the Singapore Pharmacy Council
- At least 2 years of experience and demonstrated competency, or with relevant post graduate qualifications
- Have experience in clinical patient care practice
- Strong analytical skills
- Excellent interpersonal skills in working with both internal and external stakeholders
