Job Description & Requirements
- Perform nursing assessments of Dementia Clients during admission;
- Involved in developing Individualised Care Plan for dementia Clients with other care team members;
- Involved in multidisciplinary case discussion with the other care team members;
- Conduct education and training for the dementia client’s caregiver as provides support as required;
- Conduct in-service training sessions for the staff and volunteers; and
- Oversee clients’ medical and nursing needs and maintains documentation of the Clients’ progress and outcomes of care provision.
Requirements :
· Singaporeans preferred
· Registered with the Singapore Nursing Board
· Minimum Nitec in Nursing
· Highly motivated & ability working in a team
