Job Scope:
- Coordinate and responsible for the planning, execution and evaluation of programmes and ensure smooth running of daily operations of the centres
- Activities may include Outreach and engagement of seniors within service boundary, Active ageing programme, Befriending and Buddying and Care & support services
- Develop and implement outreach plan to seniors.
- Develop and implement annual calendar plan of social and health activities for seniors.
- Ensure that the programmes and activities meet the performance indicators and operating guidelines
- Conduct needs assessment survey if necessary.
- Initiate a case record file for elderly household in the assigned territory and capture the needs of the seniors.
- Establish and motivate seniors to be active, healthy, interdependent and purposeful.
- Ensure that the activities and programmes of the centre promote a community climate of multi-racial tolerance, harmony and co-operation .
- Network with other agencies to establish additional avenues of help for the welfare of the seniors
Requirements:
- Min Degree in any field of study from a recognised institution
- Min 2-3 years of relevant community engagement / outreach support work experience from a Healthcare / Eldercare industry
- Prior relevant work experience holding supervisory position would be good
- Comfortable interacting with elderly clients
