You will be part of an integral team, tasked to centrally coordinate AIC-wide efforts for national initiatives such as Healthier SG (HSG) and Age Well SG (ASG).
- Plan and facilitate cross and multi-divisional discussion to drive AIC-wide implementation under HSG and ASG.
- Involved in discussions with MOH policy divisions on HSG and ASG and align on key expectations and deliverables.
- Serve as and be seen by the RHS and AIC divisions as the main POC for RHS on population health matters.
- Develop deep understanding of the RHS team, its strategy and plans for population health.
- Develop and implement pilot model, tools and resources that advance population health.
- Engage RHS to align initiatives with AIC strategy/framework.
- Create mindshare of population health for internal AIC divisions.
- Participate in the discussions on population health matters and bridge the gaps.
- Champion the change management efforts across AIC, including active participation in cross-divisional workgroups.
- Involved in the development of the regional orchestrator model, and operationalise on the ground.
- Facilitate the implementation and use of collaboration platforms for the sharing of learnings, best practices, information and data.
- Engage and work with internal and external stakeholders to identify gaps and barriers and work to create solutions to overcome the barriers.
Job Requirements:
- Recognized degree in any discipline, healthcare, public policy or gerontology related will be an advantage.
- Minimum 1 to 3 years working experience preferably in healthcare and/or social sector.
- Experience in maintaining strong networks and partnerships with stakeholders.
- Experience in project/programme management.
- Strong communication skills.
- Possess good critical thinking and analytical skills.
- Self-driven, with a strong team-mindset and ability to work in a fast-paced environment.
- Has the initiative and willingness to push boundaries.
- Passion in driving positive outcomes for clients and community partners.
