Job Scope:
- Carry out discharge plan assessment for inflight cases to facilitate and expedite patient discharges to community hospitals
- Provide timely information to patients and family members and assess their needs when choosing a suitable community hospital
- Document the assessment, care plans, and outcomes timely and accurately in the appropriate IT systems
- Monitor the referral trends and identify service gaps and any transfer delays
- Provide supervisory, guidance and foster collaboration among team members
- Develop career progression pathway for team members and competency test of the team
- Perform any other duties as assigned
Job Requirements:
- Bachelor Degree in Health Science / Nursing or related fields from a recognized institution
- Preferably with 5 years of experience in a supervisory role
- Strong leadership and team management skills.
- Excellent communication, presentation, and interpersonal skills.
