Residential Care Manager

Gem New Start Centre Limited

The Residential Care Manager is responsible for the comprehensive care management, guidance, and supervision of residents not limiting to ensure efficient operations.


Areas of responsibilities:

A. Case Management

Serves as liaison with other agencies, support programs, family members, professionals and significant others as needed.

1. Conduct intake assessment of referrals.

2. Handle case assignments, draft individual care plans (ICP), review case progress and determine case closure. Develop and monitor their progress relative to their ICP through all phases of the programme.

3. Facilitates implementation of individual care plan, assess and address motivational, psychosocial issues and goal setting as part of the care plan not limiting to conducts mid-term review.

4. Identify and develop additional resources where necessary that could best serve the needs of the residents.


B. Residential Care & Operations management

1. Oversee daily operations of the centre including monitoring and managing staff schedules to ensure adequate coverage and compliance with operational requirements.

2. Ensure high quality and smooth operations by reviewing internal policies and procedures, leading changes for improvements and evaluating the effectiveness of the changes made.

3. Oversee the programme needs including liaising with external partners for collaboration opportunities.

4. Lead and supervise the Residential Care Assistants in the area of home care management.


C. Programme and Training

1. Direct the planning and implementation of new programmes.

2. Work with the staff team to facilitate programme needs and conduct sessions when needed.

3. Review and develop training materials where needed.


D. Other duties as assigned.
