Job Description: UPLIFT Programme Support Personnel

Objective:The Contractor shall provide the Services by deploying 1 competent personnel (“Personnel”) to provide support for high needs and at-risk students in the UPLIFT (Uplifting Pupils in Life and Inspiring Families Taskforce) programme of Greenridge Secondary School.

Duration of Service:

  • Contract Period: From 1 Mar 2024 to 30 Nov 2024
  • Option to Extend: The Contractor grants the Authority the option to extend the Contract for a further period of 11 months (1 Jan 2025 to 30 Nov 2025).

Working Hours:

  • Mondays to Thursdays: 7:30 am to 5:30 pm (inclusive of 1hr lunch break)
  • Fridays: 7:30 am to 4:00 pm (inclusive of 1hr lunch break)
  • Service not required on Saturdays, Sundays, and gazetted public holidays

Scope of Services:

  1. Highly Individualised Support:Engage and check in with high-needs/at-risk students in timely and regular conversations.
    Conduct home visits as required.
    Act as a positive role model/significant adult for high-needs/at-risk students.
    Collaborate with parents of high-needs/at-risk students.
    Organize programs for high-needs/at-risk students.
  2. Support in 5 Areas:Academic support (low progress learners)
    Social-emotional (self-harm, trauma, mental health)
    Behavior/Attendance (irregular attendance, long-term absentees)
    Social skills (peer relation issues)
    Vulnerable (lack of adult role models)
  3. After School Engagement Programme (Gear-up Programme):Responsible for the daily operation of the Gear-up room.
    Work with members of Student Development Team (SDT) and/or Youth Worker (if any) on the after-school engagement programme (Gear-up Programme) to engage students meaningfully.
  4. Case Management:Manage the updating of case summary notes for students engaged.
    Compile cases and ensure all engagement details are recorded in the case summary.
    Promptly update or highlight to Year Heads in case of self-harm, suicide ideation, or any cases requiring immediate attention.
    Ensure the safe-keeping and confidentiality of records and details.
    Required to sign the ‘UNDERTAKING TO SAFEGUARD OFFICIAL INFORMATION’ document.
  5. Collaboration with Student Development Team:Work with members of the Student Development Team to assist Year Heads, Assistant Year Heads, HOD CCE, HOD SM, and SH SM in overseeing and completing assigned work related to supporting high-needs and at-risk students.
  6. External Liaison and Reporting:Assist Year Heads in liaising with external organizations (Family Service Centres, Community Centres, etc.) in matters related to high-needs and at-risk students.
    Forward reports, invitations, or information to Year Heads for further action.

Qualification, Skill & Experience of the Contractor’s Personnel:

  • Qualification:Minimum GCE ‘A’ Level from a local junior college, diploma from a local polytechnic, or degree in any field.
  • Skills:Ability to provide academic support (English and/or Math) for students aged 12 to 17 years old.
    Ability to connect with and engage students aged 12 to 17 years old.
    Good interpersonal, planning, analytical, and negotiation skills.
    Ability to multitask, work independently, and in a team.
    Proficient with word processing and spreadsheet software.
  • Experience:Minimum of 1 year experience in working with students aged 12 to 17 years old.
