Responsibilities
- Deliver patient-centered care with compassion and proficiency.
- Train and support caregivers in home settings.
- Administer prescribed medications and treatments.
- Offer emotional counseling to patients and families.
- Educate on symptom management and disease understanding.
- Plan, schedule, and execute effective home visits.
- Conduct assessments and document per guidelines.
- Develop, update, and assess personalized care plans.
- Provide administrative assistance to Home Care Service.
- Collaborate with care partners and contribute to research.
- Ensure quality improvement initiatives are upheld.
Requirements
- Nursing Diploma/Degree with valid Singapore Nursing Board registration.
- Minimum 5 years in Medical, Surgical, or Geriatrics.
- Palliative/Community Care background a plus.
- Proficient in clinical procedures like NGT insertion.
- Strong teamwork, initiative, and service mindset.
- Excellent interpersonal and communication skills.
- Independent and collaborative work capabilities.
- Flexibility for Singapore travel and varied hours.
