Core Function:
The Case Manager is responsible for providing casework services for clients of SUN-DAC and their families.
Core Responsibilities:
1. Provides information and referral, consultation, counselling and groupwork programmes for clients and caregivers.
2. Uses an evidence-based approach for effective delivery and evaluation of casework services for clients with disabilities.
3. Monitors, evaluates casework programmes and ensures all programmes are able to meet stated objectives and outcomes.
4. Develop long term care plans for caregivers.
5. Submits timely assessment and evaluation reports of clients.
6. Interprets the vision and mission of SUN-DAC to the community through direct involvement with public and private groups and networking with other social service agencies.
7. Assists in conducting staff trainings and supervision.
8. Organise and/or conduct caregiver trainings.

Core Work Requirements:
Education:
· Bachelor’s degree in social work, psychology, or related field.
Experience:
· A minimum of three years of experience working in the sector preferred, fresh graduates are welcome to apply.
· Must be computer proficient.

· Demonstrated skills in organization, management and leadership.
· Knowledge of and ability to work effectively with the community and stakeholders;
· Excellent oral and written communication skills.
· Ability to establish and maintain positive working relationships with staff, volunteers, and the public. Must be self-directed and work independently.
Physical Requirements:
Must have the ability to attend community meetings, training events and agency meetings.
Working Hours:
Full time; 40 hours per week; with the ability to work flexible hours depending on needs and events
