Job Role Description
Case Management
- Provides holistic services that include interviews, assessments, case formulation, intervention planning, implementation of evidence-based practices, and evaluation of outcomes.
- Works collaboratively with caregivers, colleagues and/or professionals from relevant agencies to address the needs of students from diverse backgrounds using a family-centred model of care.
- Establish rapport, engage parents and conduct holistic bio-psycho-social assessment of child’s and family’s needs, risks and strengths using the CANs tool
- Conduct parents’ meetings to share assessment outcomes of child and family and recommendations for family care plan
- Facilitate family’s understanding of needs of child and programmes that can be offered to better support the child and family
- Conduct home visits as part of assessment or intervention follow-up
- Evaluate and identify areas for follow up (eg referral to internal and external community resources)
- Conduct inter-agency case conferences
- Assist parents with applications related to financial assistance, schemes or programmes offered in the community
- Experienced in case management
Group Work
- Develop groupwork/ programmes to support family needs
- Facilitate and conduct group work/ programmes
Systems Development
- Contributes to workflow reviews and updates when necessary.
- Works collaboratively with the Senior Social Worker to develop standard operating procedures for the Social Work unit.
People Development
- Enhance professional growth and development by keeping up to date with current literature and through participating in educational programmes, in-service meetings, and workshops.
- Translate evidence-based research into practice.
- Provide workshops to staff of The Village
- Provide training, peer support and/or supervision to junior/new staff such as Social Work Associates
Documentation
- Complete documentation (eg case notes and social report) accurately and promptly
- Maintain accurate records, prepare reports for governing agencies, collate information
Collaborative Community Network
- Demonstrate knowledge of available community resources
- Engage and work collaboratively with various community agencies (eg Family Service Centres, Social Service Office or other Social Service Agencies to ensure holistic case management for child and family)
Education and Experience
- Minimum Degree/Post Graduate Diploma in Social Work
Technical Skills and Competencies
- Minimum 3 years of experience working with children and families preferred
- Have good understanding of diverse cultures and races
- Is able to communicate using more than 1 language
- Registered or Accredited Social Worker
