JOB SUMMARY
The incumbent will provide active ageing activities, befriending services for vulnerable seniors who require additional social support; and referrals for care services when required. He or she works closely with the Centre Supervisor to promote healthy living and keep seniors engaged within the community which includes regular monitoring of seniors' health condition.
KEY DUTIES AND RESPONSIBILITIES
- Assist Centre Supervisor to conduct active ageing activities and organize special events (i.e. National Day, Christmas, Chinese New Year) for the seniors and their caregivers.
- Conduct community outreach and needs assessments to seniors at a designated service boundary.
- Provide befriending services to vulnerable seniors including connecting them to care services when necessary.
- Collaborate with Healthier SG ambassadors and the local community nodes to implement social and lifestyle interventions to help seniors age in place.
- Recruit and train volunteers to assist with the active ageing activities and outreach services.
- Perform administrative and reporting duties such as daily programme attendance, health screening recording, client register, and other data and statistics rquired by the authorities.
- Assist in monitoring Alert Alarm System wherever available.
- Perform other duties as assigned by the reporting officer.
JOB REQUIREMENTS
- GCE 'O' Level / Diploma
- 1-2 years of relevant experience in elderly befriending service
- Have interest and passion to work with seniors and their caregivers
- Good organisational skills, team-oriented with excellent interpersonal and communication skills
- Knowledge in eldercare is an advantage.
