Job Summary:
· Right site patients and achieve optimal utilization of SLH services
· Ensure expedient processing and managing all applications for admissions in a timely manner
· Implement policies and procedures that improve efficiency of the Referral Centre
· Support joint initiatives and collaborative projects undertaken with other healthcare and community partners
Responsibilities and Duties:
· Conduct screening of referrals and assess suitability for transfers to SLH
· Participate in clinical and/or quality improvement initiatives
· Review and implement clinical protocols, processes, procedures and policies
· Collaborate with other hospitals and strategic partners to develop and refine workflows to ensure seamless and expedient transfers
· Participate in new projects and initiatives, where there may be impact on clinical services of the hospital (inclusive of providing administrative support in clinical research)
· Assist in the preparation of reports and statistics as required
· Assist in identifying problem areas, recommend and implement improvements
· Perform other duties as assigned
Job Requirements:
· Minimum qualification of a clinical degree or equivalent, preferably with at least 5 years of work experience in geriatric setting.
· Able to work independently
· Good collaborative and communication skills
· Demonstrated ability and interest in working with the elderly population
