- Assist in the development of strategies and implementation plans for new initiatives to strengthen community network for seniors.
- Assist in the review of care and support protocols to ensure case processes and standards are established across Satellite Offices i.e. triaging, case sensing, referral and coordination.
- Support capability building and professional development efforts to equip Satellite Office (SO) staff with necessary competency to perform care coordination roles.
- Implementation of Community Monitoring System (CMS) to all SOs to support vulnerable seniors who live alone or with spouse only and presented with risk factors.
- Established robust referral protocols between Silver Generation Office (SGO) and community partners to ensure that coordinated supports are rendered to the vulnerable seniors in addressing their risk and needs.
- Implementation of refreshed SGO Care and Support Guide and Risk Stratification Protocols to guide SOs in the care processes.
- Support the implementation of Vulnerable Adult Screening Guide (VARG) by MSF to SOs to ensure the prompt response to support vulnerable adult who are subjected to abuse or neglect by their caregivers.
- Support the setting up of Community of Practice (CoP) to facilitate collective learning on social and health issues which have an impact on the vulnerable seniors in the community.
Job Requirements
- Degree in any discipline, healthcare, social work or counselling related field will be an advantage
- At least 2 years of relevant experience, preferably with experience in healthcare, social services or community engagement. Candidates with no relevant experience may apply too
