Overview
- Oversee both in-patient and out-patient discharges.
- Evaluate the medication charts or prescriptions of each patient to assess the suitability of prescribed drugs and communicate with the prescribing doctor for necessary interventions. This review ensures that all orders are clear, and prescribed doses fall within therapeutic limits, while also identifying potential adverse reactions and interactions.
- Accurately and promptly fulfill all drug orders.
- Verify and dispense prescribed drugs with counseling.
- Ensure compliance with legal requirements in all sales and supply of medicinal products, maintaining records as mandated by the hospital and/or legal authorities throughout the drug receipt, supply, storage, and distribution phases.
Requirements
- Registered with Singapore Pharmacy Council.
- Minimum 1 to 2 years of working experience as a hospital pharmacist.
