Job Title: Patient Service Executive / Network Clinic Coordinator


Responsibilities:


· Support United Primary Care Network (PCN) recruitment, onboarding, and training of GP Partners.

· Promote GP engagement, attend to their enquiries and support them in Healthier SG (HSG) drive.

· Work closely with GPs for PCN and HSG enrolment of chronic and well patients

· Liaise with the service providers or GPs in arranging provision of ancillary services by PCN/HSG

· Support GPs in better integration and coordination with various social partners and institutions

· Provide administrative support to GPs on patient referral to community and social partners, such as Active Aging Centres (AAC), Day Care, Home Care services etc.

· Support the compliance of GP partners with audit requirements of the PCN program.

· Establish feedback management from MOH/AIC to GP partners and vice versa.

· Support the admin team in administering surveys and collating data for analysis and reporting.

· Share Clinic Performance Management to GP partners.

· Assist Nurse Counsellors to provide basic counselling to patients on their care plan compliance and lifestyle modification as required.

· Perform adhoc duties assigned by the Head of PCN.


Job Requirement:


· GCE O Level/ Diploma Holder/ Degree Holder.

· Relevant work experience in healthcare would be an added advantage.

· Ability to work as a team player and independently with minimal supervision.

· Meticulous and able to keep proper and timely documentation of HSG/PCN records, database and quality-related reports.

· Competent in MS Office, in particular MS Excel.
