Job Description:
• Perform relevant clinical assessment and evaluate the patient’s current disease control.
• Assess patient’s concerns about his/her condition and learning needs.
• Explore for possible precipitating factors for sub-optimal/poor control.
• Reinforce patient/caregiver’s education and self-management skills.
• Develop/evaluate the plan of care to improve disease control with patient/caregiver.
Job Requirements: Min Degree in Nursing & Advanced Diploma in chronic disease / mental health / endocrine / diabetes / gerontology or similar
