Job Scope:
- Trained in chronic disease management in primary care setting
- Work closely with doctors, advanced practice nurse, allied health members to manage chronic conditions
- Obtain a thorough health history and assess your level of disease control
- Identify factors that contribute to your sub-optimal disease control
- Provide education on lifestyle modification and teach self-management to empower you to manage your health
- Monitor your condition and work with your doctor to review your plan of care regularly.
Job Requirements:
- Min. Diploma/ Degree in Nursing
- Possess Advanced diploma/ specialist diploma in either chronic disease/ mental health/ endocrine/ diabetes/ gerontology or any other relevant field
- Valid SNB & BCLS
