Primary Objectives of Position:
- Increase public awareness on dementia / mental health by organising outreach events to provide information and education
- Engage the beneficiaries through various activities and programmes targeting their holistic health & well-being
- Provide basic emotional support, education, service linkage, care coordination and follow-up to clients, including caregiver support
Job Responsibilities:
- Coordinate & support outreach events in community to promote mental wellness, health awareness and information
- Engage community partners such as grassroots to raise awareness, education and training on mental health and ensure smooth conduct of these events
- Conduct targeted mental health and dementia case finding/screening to promote early identification and facilitate timely support
- Organise structured preventive activities and/or programmes for the community, those at risk of mental health conditions and clients
- Carry out case assessments/re-assessments, provide basic mental health information and emotional support to seniors & caregivers, via phone-call and/or home visits as required
- Provide information & referral to healthcare or social services, such as preparing the necessary resource materials to be provided to clients and caregivers
- Initiate and support group work and support groups to meet the needs of dementia/mental health clients & caregivers
- Facilitate communications between the programme and client/family/caregivers and/or the partners
- Keep and maintain client records and programme data and documents, in accordance to programme requirement, stakeholders reporting and prevailing personal data protection security guidelines
- Support in programme development, monitor trends and issues relating to dementia/mental health services and needs of clients and their families
- Extend support by contributing in different projects and committees from time to time, as and when required
