SAMH manages the Activity Hub, where psychosocial rehabilitative programmes are conducted to assist residents in their recovery from mental illness and to reintegrating them into the community. Activity Hub’s services include occupational therapy, social work, psychological services, and vocational placements.

Purpose
Promote residents’ quality of life and reintegration to the community by focusing on the areas of vocational, activities of daily living and leisure activities
Core Duties:
1. Clinical Work
- Conduct therapeutic group work and track outcomes and collect data to ensure effectiveness
- Conduct individual / group sessions for skills training, psychoeducation and support (such as budgeting/community resources/conflict resolution, etc)
- Conduct home visits where necessary
- Conduct needs analysis and expand the types of volunteer opportunities for residents
- Work collaboratively with OTs / Psychologists / Vocational Placement Officers towards developing intervention and discharged plans for ICPs
- Make referrals to other services when appropriate
- Share new practice knowledge with other to encourage a culture of learning and continuous improvements
For Senior position:
- Lead and conduct case assessment and risk assessment for complex cases with high risk
- Ensure the case work management meet the requirement of MSF
- Review social reports and other documents prepared by social/case workers to ensure quality standard
- Develop / Fine tune service standards, performance indicators and operating guidelines of the department in consultation with RO
- Provide supervision to team on regular basis in their professional development
- Supervise NUS / SUSS social work students on placement
2. Administrative
- Ensure proper maintenance, collation and timely submission of statistics / period reports / annual reports to funding bodies, external agency and SAMH management
- Attend and present regular case conferences and case discussions (eg. ICP)
For Senior position:
- Lead, design and conduct research on current programmes / specified issues
3. Others
- Assist in daily operations eg. Incident reporting, crisis management, sending and receiving residents, business contingency plan
- Organize social and recreational activities to improve the quality of life of the residents
- Network and collaborate with stakeholders, including all Homes @ PV
- Conduct relevant training programmes of staff and external parties
- Deliver lecture / workshops / talks on mental health related topics as and when required
- Perform any other duties as assigned by the organization, which at times may require to work beyond normal work schedule
Qualifications
- Degree in Social Work/Psychology or Grad Diploma in Social Work
- Some experience in relevant field
- Self-motivated and self-directed
- Proactive
- Good Communication skills and resourceful
- Team spirit
- At least 3-5 years of direct practice experience (For the senior position)
