REHABILITATION COUNSELLOR - EMPLOYMENT SERVICES

JOB SUMMARY


The Rehabilitation Counselor supports people with mental health issues to gain and sustain employment and/or reemployment in the competitive job market according to the principles of psychiatric rehabilitation. The incumbent is expected to do case management work, counsel his or her clients in the course of the service, and liaise with caregivers and/or other community services that the client is connected to. The incumbent is also expected to assist in the employment training and provide support to the peer support network in the service.


KEY DUTIES AND RESPONSIBILITIES

  1. Provides case management services to the centre's clients which includes intake assessment, vocational readiness assessment, individualized care plans, counselling, and other clinical best practices relevant to clients’ recovery goals.
  2. Assist clients in creating strategies to develop their strengths and adjust to their limitations at workplace.
  3. Monitor clients’ progress and adjust the rehabilitation or care plan as necessary.
  4. Timely submission of clinical documentation, ensuring quality record and data integrity.
  5. Be involved in case supervision, case presentation and case conference.
  6. Help employers understand the needs and abilities of people with mental health issues (PMHI), as well as laws and resources that impact PMHI when the opportunity arises.
  7. Support and contribute in effective change management, and strengthening collaborative network within the social service/mental health sector.
  8. Responsible for his or her professional development which includes training plans, cross functional involvement, and discussing with the Reporting Officer on career pathway.
  9. Any other related duties and functions assigned by the Reporting Officer.

JOB REQUIREMENTS

  • Degree in Counselling / Psychology / Social Work
  • 2 years of relevant experience
  • Passionate in people work
  • Ability in case management work and counselling preferred
  • Knowledge of mental illness, and social service is an advantage
  • Effective team player and good interpersonal skills
  • Good problem solving skills and advocate for mental health causes
