Duties & Responsibilities:
- Provide direct nursing care to residents, including assessment, monitoring, and evaluation of their physical, emotional, and social needs.
- Collaborate with residents, families, and interdisciplinary teams to develop individualized care plans that address residents' specific health needs and promote their well-being.
- Maintain accurate and up-to-date documentation of assessments, care plans, medication administration, progress notes, and relevant communications.
- Advocate for residents' rights, dignity, and autonomy throughout the care process.
- Collaborate effectively with interdisciplinary teams, including physicians, therapists, social workers, and case managers, to ensure holistic and coordinated care for residents.
- Educate residents and their families on health promotion, disease prevention, and self-care techniques.
- Adhere to safety protocols, infection control measures, and regulatory guidelines to maintain a safe and clean environment for residents and staff.
- Foster positive relationships with residents' families, keeping them informed about their loved ones' health status, care plans, and progress.
- Stay updated with current knowledge, best practices, and advancements in nursing care, geriatrics, and relevant healthcare regulations.
- Provide emotional support to residents and their families during times of distress, grief, or end-of-life care.
Requirements:
- Degree or diploma in Nursing from an accredited institution.
- Registered Nurse from SNB
- Relevant experience as a staff nurse, preferably in a long-term care or geriatric setting.
- Strong clinical assessment and critical thinking skills.
- Knowledge of nursing best practices, standards, and regulations.
- Excellent interpersonal and communication skills, with the ability to establish rapport with residents, families, and interdisciplinary teams.
- Compassion, empathy, and patience in working with residents with diverse physical and cognitive abilities.
- Ability to work effectively in a fast-paced environment and handle multiple responsibilities.
Working Hours & Location
- West Area, West Coast
- Up to 44 working hours per week
- 4 days work week, including weekends and public holidays.
