- This role is similar to case management – managing the care plan for the patients
- You will be the key person who will make the most difference as you make home visits to provide palliative care and recommend psycho-social support to our terminally ill patients and their families.
- You will be part of the most dedicated, compassionate, and caring teams comprising doctors, nurses, and medical social workers providing all-in-one professional care to patients at home at all hours, seven days a week.
