Responsibilities
- Conduct case assessment and formulate Individualized Care Plan for Persons with Dementia and their carers.
- Provide intervention such as psychoeducation, supportive counselling and service referral. Able to administer Advance Care Planning will be a bonus.
- Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
- Provide case recordings and relevant documentations.
- Participate in team meetings and case conferences as required.
- Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
- Plan and facilitate Peer Support Group for Carers of Persons with Dementia.
- Plan and set-up Peer Support network amongst carers for Persons with Dementia.
- Support in outreach events and talks.
- Any other duties as assigned.
Requirements
- Preferably with degree in an allied health profession/social work
- Good oral, written and interpersonal communications skills
- Ability to speak a second language and/or dialect(s) will be an added advantage
- Ability to facilitate group sessions will be an added advantage
- Able to work on occasional Saturdays and weekday evenings.
Competencies:
- Passionate about dementia care and working with people
- Prior working experience in dementia care will be an added advantage
