JOB PURPOSE
The social work team provides a critical communication link between the Home and our resident’s next of kin. To provide assessment, rehabilitation and supportive care in line with policies and guidelines. Practise and be involved in case management, working Agencies, Centres and Organisations as well as individuals and families, and as assigned by the Director, Care Services or Head of Home, Red Cross Home for the Disabled.
The position is a key management position within the Home and reports to the Head of Home and, in his or her absence, the Director, Care Services.
PRINCIPAL ACCOUNTABILITIES
- Operation
- Manage referrals for admission to RCHD, including the DAC (Day Activity Centre)
- Plan for admissions, transfers and discharge
- Arrange for client assessments before admission
- Coordinate documents for approval
- Participate (as an interdisciplinary team) in client care planning
- Conduct periodic reviews of cases
- Counsel clients, caregivers and families when required
- Communicate and consult in confidentiality with internal and external parties on matters related to client’s care and activities
- Liaise with other internal or external support services and agencies when necessary
- Advocate for and protect client’s rights
- Manage clients' financial, social, and emotional needs.
- Manage transport and service needs of the clients.
- Manage environmental and personnel safety and hygiene
- Source for funding opportunities and submit applications where applicable.
Customer Management
- Interview client, potential clients and the caregiver/family members
- Work with the Executive and Administrative Assistant to follow-up on Home fees and arrears
- Coordinate with the relevant agencies on matters related to RCHD and DAC. These are primarily, but not limited to, SG Enable, Ministry of Social and Family, government-restructured hospitals, AIC, etc.
- Be the primary conduit for communication between the Home/DAC and the client’s legal guardians/family members
- Encourage strong volunteer involvement among individuals, groups and companies
- Assist in receiving and attending to visitors to RCHD
Administration
- Carry out periodic reviews of statistics from social and financial reports
- Prepare reports for statutory submission and statutory meetings
- Ensure timely submissions of funding, statutory submissions and reports for meetings
Supervision
- Provide on-the-job training and coaching
- Provide training on CAF
- Oversee the reissuing of MFEC cards whenever they expire.
- Supervise the coordination and management of Social Work Associates.
- Stimulate and motivate staff to achieve divisional goals and objectives
Any Other Duties
- Deputise the Head of Home or Director in his/her absence and when assigned
- Participate in fire drill exercises in accordance with the Fire Safety Bureau Regulations with the assistance of all staff concerned
- Assist in events held at the Red Cross Home for the Disabled
- Organise talks from time to time, which may be beneficial to both the home and assist with issues such as end-of-life questions, costs of funerals and any legal issues that may occur when the client’s guardian passes.
- Participate in training and team meetings
- Promote the Red Cross Home For The Disabled to ensure optimal usage of resources
- Promote the Day Activity Centre to ensure optimal use of resources
- Recruit volunteers for RCHD and (Individuals and Corporate)
- Undertake any other duties as assigned.
- To undertake one process improvement project for the Social Work Team every year and to present the findings in Q4 of every year.
JOB SPECIFICATION
Qualification
- Possess a recognised university degree in Social Work or and /related subject
Experience
- For graduate, at least 2 year of relevant experience
- For non-graduate, at least 3 years of relevant experience
- Experience at other Nursing or Disability Home would be an advantage.
- Masters in Counseling and/or specialised certification in therapy is advantageous.
- Prior experience in providing case and clinical supervision to social/case workers.
- Proficient knowledge and experience in social work interventions, social policies, and community engagement.
- Demonstrated leadership experience in leading teams, monitoring staff competency, and overseeing training.
Additional skill and knowledge
- Computer literate
- The ability to speak a few languages or dialects would be an added advantage
- Good knowledge and understanding of local and international community welfare programmes and services
