Job Purpose
The Medical Social Worker works closely with Agency for Integrated Care (AIC) on referrals from hospitals and government-linked agencies and other relevant stakeholders to support aging in place for patients and their families in the community. The incumbent is responsible to support a coordinated approach with multi-disciplinary stakeholders to regularly review bio-psycho-social-spiritual aspects to ensure holistic care.
Job Responsibilities
- To provide psychosocial and financial assessment, coordinated case management and interventions to enable the patient/client to function optimally in the home environment and community at large
- Participate in multi-disciplinary team meetings in developing individualised care plans and facilitate family conferences for patients/clients and family members together with members of the multi-disciplinary team for facilitation of effective communication
- Assess caregiver stress and burden and provide supportive platforms and initiatives for caregivers
- Assessment of risk and to intervene in a timely manner
- Information and referral services for patients and family members as deemed necessary for patient/client’s optimal functioning in the community with relevant resources
- Undertake projects and any other duties as assigned by Manager-Social Work or Head of Centre/Centre Manager
Job Requirements
- Master/Bachelor/Post Graduate Diploma in Social Work
- Preferably a registered Social Worker with the Singapore Association of Social Workers (SASW)
- At least 2 years of relevant working experience in a healthcare or eldercare setting
- Any deployment as designated within MWS Integrated Resource Group Social Work - Community Eldercare Services
- Good team player with positive attitude, caring and compassionate individual
- Good interpersonal skills, able to communicate people at all levels such as patient’s families, government agencies and other professionals
- A quality service mind-set
- Basic computer knowledge, familiar with MS Office tools and Internet
- Familiarity with dialects will be an advantage
- Advance Care Planning facilitator would be an advantage
- Inclination towards palliative care and/or community case management will be an advantage
- Ability to speak and write effectively in English.
