Woodlands Health encompasses acute and intermediate as well as long term care facilities. The Senior / Social Work Assistant has an important role in supporting the Medical Social Work Department in the provision of the continuum of care services within the campus and into the community.
The Senior / Social Work Assistant will work together with the Medical Social Worker in the provision of psychosocial care to the cases referred to the Medical Social Work Department. They will provide casework management and provide support in therapeutic groups in collaboration with the Medical Social Worker across the various services provided within the campus.
Responsibilities
- Provide psychosocial care to patient and their significant others in Singapore's multi-cultural context in collaboration with the Medical Social Worker
- Review interventional plans and work with the management of the case together with the Medical Social Worker
- Conduct independent assessment of straight forward and non-straight forward financial cases and apply for relevant national financial schemes for patients and family members
- Prepare brief social assessment for the Medical Social Worker for continuation of follow up
- Conduct home visits and organisation engagement independently and provide assessment of the home situations or organisations to the Medical Social Worker
- Support programs planning and support groups that are run by the Medical Social Work Department both on campus and within the community.
- Meet the department clinical indicators and be part of member of Quality improvement related projects that will improve patient's outcome
Job Requirements
- Process a relevant diploma, preference would be given to those who holds a Diploma in Social Work or Psychology or Counselling or Social Services
- Preferably with 2 years of relevant experience or healthcare experience.
