Woodlands Health Campus (WHC) Rehabilitation Department is looking for an enthusiastic and motivated Senior Physiotherapist to join our evolving team. This position is suitable for a person who is interested in community hospital services, transitional care, involve in ageing in place strategies as well as community wellness programs such as driving health literacy and maintaining an active lifestyle to improve one’s personal health and wellbeing.
The candidate will spend his/her time at both hospital inpatient setting as well as the community setting specifically looking at how to improve the continuity of care from hospital to home. He/she will work closely with our various clinical teams, comprising of our doctors, nurses and allied health professionals involved in integrated care services, community services as well as with our community rehab services based within the hospital. A large part of this role will be to deliver care to patients following an acute episode or entry into our health system. The daily work would include clinical work in an inpatient setting as well as at the home setting with some administrative duties related to the clinical practice as well as delivery. The person will also help the inpatient in charge in deriving the integrated care process that transits from acute to community hospital and into the patient’s home, to deliver excellent quality of care geared towards enabling support and self-care in the community.
The position also requires the candidate to possess good communication, interpersonal and organizational skills while being able to manage his/her own time effectively. In addition, the candidate should also be a good team player and willing to continuously improve his/ her knowledge in population health and geriatric related areas. The person will be involved in community engagement or education in driving program towards active ageing for the population living in the north. The Requirement
- Degree (BSc Hons Physiotherapy) or equivalent recognized by the Allied Health Professions Council (AHPC)
- Registered with the Allied Health Professions Council.
- Patient-focused, and demonstrate sound clinical knowledge and reasoning, critical thinking, judgment and decision-making in the provision of care
- Team player, with good interpersonal and communication skills
- Candidates with 3 or more years of relevant work experience in a hospital or clinical setting
