Social Worker/Social Programme Executive (Flourishing Minds)

Develop and conduct mental wellness programmes for CYP and their families as well as provide coordinated person-centric care through case management and interventions to CYP who have mild mental health needs. The purpose is to support the CYP’s mental wellness journey.


Job Description:

Tier 1: Programme Management (~80%)

  • Conduct environmental scans, assess client needs and provide recommendations.
  • Develop, implement and evaluate programmes for CYP aged 10 and above, parents and professionals based on their needs.
  • Identify areas of improvements in programmes and implement quality control and compliance procedures to ensure relevance and effectiveness.
  • Maintain regular records on progress of beneficiaries.
  • Develop information materials for distribution.
  • Coordinate with stakeholders (i.e. schools, community partners, volunteers) for the execution of programmes.
  • Conduct outreach activities or develop outreach plans to target client groups, other organisations and the community.
  • Opportunities to be involved in organisational level projects.

Tier 2: Casework and Counselling (~20%)

  • Formulate, develop, implement, and review intervention plans with CYP and/or caregivers.
  • Provide psychoeducation, and basic psychosocial and behavioural interventions.
  • Maintain up-to-date case documentation.
  • Conduct risk assessment, safety planning and implement safety plans as appropriate.
  • Involve parents and peers in the youth’s mental wellness journey.

Requirements

  • Degree in Psychology, Counselling or Social Work.
  • At least 2 years of relevant work experience in social service sector.
  • Experience in groupwork facilitation, counselling or working with CYP mental health needs will be of added advantage.
  • Possess ability to perform basic research and evaluation.
  • Must be an insightful and reflective team player with passion in CYP mental health, strong oral and written communication skills, effective interpersonal skills, high levels of initiative, adaptability and responsibility, and willingness to grow professionally with the organisation.
