Overview
Reporting to the Pharmacy Section In-Charge, you will help provide safe, efficient and timely distribution (dispensing) of drugs in NUH. You will coordinate activities in your area of purview to be in line with the objectives of the pharmacy department.
Job Description
You will be responsible for the following:
· Review written order form (medication chart, medication record or prescription) to determine the drug needs of the patient, ensuring that overdoses have not been prescribed, that no toxic reactions will result from administration to the patient and that all drug orders are specific (contact physicians as necessary to review ambiguous or illegible prescriptions)
· Fill prescriptions as ordered by attending physician by:
I. Filling bottles, packaging material, dosage form and other packaging forms with measured amounts of medication in accordance with prescription specifications
II. Documenting supplies used and labelling containers (including directions for use)
· Supervise the compounding and/or dispensing of medication (unit dose, intravenous admixtures, investigational drugs and outpatient prescriptions)
· Explain directions for prescription use (drug therapy) to patients
· Monitor response of patient to prescribed drug therapy
· Make recommendations about appropriate drug therapy to doctors
· Carry out medication reconciliation when appropriate
· Report adverse drug reactions encountered by patients
· Provide information to other health professionals
· Ensure patients receive their medicines correctly in a timely manner
· Informs prescribers of any drug interaction, adverse drug reactions, contraindications
· Maintains a shelf inventory of medications and pharmaceutical supplies, ensuring that storage is in accordance with security, environmental and product shelf life parameters
· Implement and adhere to the policies, practices and regulations established by the Pharmacy Department, various NUH management committees and outside regulatory agencies pertaining to pharmacy
· Perform inspections of drug storage areas throughout the hospital and performs other job-related duties as assigned
· Treat each patient, visitor and employee with compassion, courtesy and tact
· Assist in acquisition of special items for patients with approval from senior pharmacist
· Any other duties or responsibilities as assigned
Job Requirements
· Bachelor of Science in Pharmacy from an accredited college by Singapore Pharmacy Council.
· Registered with the Singapore Pharmacy Council
· Able to work independently and in a team.
· Good listening and communication skills.
· Possess basic supervisory and management skills
· Flexibility to work on weekends/public holidays occasionally.
