Social Work Associate (Sunbeam Place)

To provide the best possible preventive, developmental, remedial and rehabilitative interventions unique to every child residing at Sunbeam Place (SBP). To deliver quality trauma-informed programs, case management and counselling services to meet the objectives of Home-In-Home care model.


Job Responsibilities


General Duties

  • Work in partnership with the SBP team.
  • Work collaboratively with professionals from respective social work organizations, the MSF, educational institutions, hospitals and any other organizations involved in the care plan of every child.
  • To participate in the Children In Care Standing Committee (CICSC), Admission, Progress and Discharge Workgroup (APDW) and Care Plan Meetings, as required.


Case Management

  • Co-manage up to 6 low risk cases (i.e. CANS Low Needs) from different levels/houses.
  • Provide counselling support to assigned residents by regularly meeting them at least once a month and ensure that these sessions are maintained in written records in a timely manner.
  • Co-conduct and assess residents’ overall wellbeing and formulate/implement a care plan to monitor the progress and development of the resident.
  • Monitor and record changes in resident’s behavioural performance in care plan and provide updates during care plan meetings or when necessary.
  • Work with the Residential Care staff to ensure the implementation of care plan interventions.
  • To regularly evaluate the outcome of care plans and any other intervention carried out for the residents.
  • Maintain and update records regularly.


Family Engagement and Reunification

  • Co-work with Social Worker to propose and implement permanency plans for residents who are unable to be reunified to identified carer or who had unsuccessful attempts of reunification back to carer.
  • Co-work with Social Worker to counsel residents and family members/significant adult to facilitate implementation of care plan and permanency plan.
  • Work with various stakeholders & community partners (e.g. MSF & FSC) to plan and work towards family reunification.
  • Co-conduct home visits when necessary (especially when residents are on home leave and on extended home leave).
  • Update residents’ family on their performance in the Home and propose reintegration to their families when applicable or appropriate.


Case Management Administration

  • Support in the administrative matters of the Home and casework teams such as the collation, submission of reports as well as maintaining the casefiles storage and database.
  • To prepare admission documents and casefiles, access files for new admissions.
  • To ensure database is updated timely and accurately on a monthly basis.
  • To ensure proper storage of casefiles.
  • To submit accurate and timely monthly reports to MSF.
  • To prepare the SBP monthly reports for CICSC reporting.
  • To support in the application of The Straits Times School Pocket Money Fund (SPMF).
  • To keep track and maintain office supplies.
  • Any other admin tasks assigned by Reporting Officer


Post Discharge Follow-up

  • To follow up on discharged cases for as long as it is possible till the age of 21 years old.
  • To implement relevant outreach or intervention programmes for SBP’s ex-residents.


Resident Management

  • Co-lead with Residential Care staff to ensure the resident’s safety, health & positive behaviour when they are in the Home and during outings & appointments.
  • Monitor the residents to ensure that they are present in SBP, house or the activity you are supervising (or their whereabouts are known to you when they are not at the Home) and double check, communicate and give permission for residents to leave areas or activities under your supervision when it is requested or required.
  • Monitor & report to the Management of SBP of any signs or suspicions of physical, sexual or emotional abuse or self-harm.
  • Supervise residents and teach rules and provide reminders about safe touch & unsafe touch; and check in with residents to flag any behavioural concerns and issues.


Job Requirements

  • Diploma in Social Services/ Social Work.
  • Candidates possessing relevant diplomas/degrees may be considered.
  • Preferably one year's experience in a social service setting.
  • Fresh Graduates may apply.
  • Able to work independently.
  • Agile and flexibility in working with children aged between 7 and 21.
  • Good team player with a high level of initiative and independence.
  • Excellent interpersonal communication skills.
  • Able to work after office hours, when necessary.
