Job Description
- Ensure the safety and dignity of each Jaga-Me’s patient is upheld at all times
- Escalate any problems encountered in a patient's condition and/or in the handling of patients and/or caregivers to respective departments
- Work closely with the Medical Director in carrying out home medical/nursing care
- Provide caregiver training, wound care, nasogastric tube management, PICC line and port-a-cath management
- Initiate nursing care plan and implement management measures
- Refer patients to other healthcare providers and professionals such as inpatient hospice and specialized counsellor when the need arises for respite care or symptom control
Job Requirements
- Registered Nurse with Singapore Nursing Board License
- Minimum 3 years of experience working in Restructured Hospital or Home Care setting
- Preferably with Palliative Care/Gerontology/Oncology training or background
- Able to execute with dedication and teamwork
- Excellent people skills to navigate challenging situations and facilitate alignment of different priorities
- Excellent communication skills with a strong sense of empathy
- Able to commit to standby for cases if rostered on weekdays from 6pm to 8pm and weekends from 8am to 2pm with overtime pay
- Those who are competent in IV cannulation and blood taking procedures and able to work night shift from 8pm to 8am are encouraged to apply for permanent full-time/part-time or locum positions.
