You will work closely with our team of healthcare professionals in providing quality clinical care, home care, as well as mobile health screening services.
- The home care nurse’s responsibilities include performing patient visits and monitoring their conditions, conducting health screening and nursing care- e.g. administering IV, changing dressings, cleaning wounds.
- Assess and chart observations of the patient’s condition at each visit.
- Complete evaluation tasks, including reviewing medication and vital signs.
- Dress or redress wounds and assess the healing progress.
- Traveling to patients’ homes and managing their care plans according to physicians’ instructions.
- Educating caregivers and family on the aftercare or ongoing care of the patient
- Responsible for the safe delivery of care and clinical decisions for all home care clients.
- Render clinical care and ongoing evaluation of care provision for home care services.
- Providing nursing services at in-house and industrial clinics.
- Maintain proper and accurate nursing documentation for clients’ clinical records.
- To carry out their responsibilities in accordance with the Nursing Home Services guidelines, protocols and standards operating procedures (SOPs).
- Any other ad hoc duties.
- Diploma in Nursing or equivalent and recognised by the Singapore Nursing Board.
- Registered with Singapore Nursing Board is preferred.
- Post Basic and Degree in Nursing.
- Experience with advanced wound management is preferred.
- Has a Valid BCLS & AED Cert.
- Able to perform phlebotomy.
- Willing to travel.
