Job Description

  • Screening of patients for psycho-social, environmental, vocational and financial needs
  • Assess the patients and families’ ability in carrying out health behavior change and provide continuous support and motivate patients to achieve health goals.
  • Working closely with the care team to provide patient’s care plan. Eg. Lifestyle and behavioral health modification
  • Engage, Educate and Empower patients and families in their care
  • Educate patients and families to understand how their behavioral health factors can affect their health and illness
  • Participate in community outreach programs
  • Other adhoc duties assigned

Requirements

  • Candidate must possess at least minimum bachelor degree in Psychology/Social Work
  • Good interpersonal skills , strong organizational skills and case management experience
  • Able to commit on every Saturday
