Job Description
- Screening of patients for psycho-social, environmental, vocational and financial needs
- Assess the patients and families’ ability in carrying out health behavior change and provide continuous support and motivate patients to achieve health goals.
- Working closely with the care team to provide patient’s care plan. Eg. Lifestyle and behavioral health modification
- Engage, Educate and Empower patients and families in their care
- Educate patients and families to understand how their behavioral health factors can affect their health and illness
- Participate in community outreach programs
- Other adhoc duties assigned
Requirements
- Candidate must possess at least minimum bachelor degree in Psychology/Social Work
- Good interpersonal skills , strong organizational skills and case management experience
- Able to commit on every Saturday
