Provide Centre-based Nursing and Nursing Assessment and management at St. Andrew’s Senior Care (Tampines Central)
Job Description
- To provide nursing care to the daycare clients. Tasks can include: prepare client’s assessment and clinical documentation, medicine packing, wound care, centre-based nursing, updating caregivers on client’s condition and carrying out competency audits on healthcare assistants and community care associates.
- Monitor outcomes of services and perform escalation of cases if there is a need for more acute management of clients;
- Implement nursing interventions as outlined in the care plan and carry out nursing procedures according to the care needs of the client;
- Co-ordinate with other team members and update them on the changes in the client's condition and needs;
- To be in-charge of all nursing procedures and inventory in the centre.
- To be able to conduct centre audits to ensure good infection control and centre processes are in place.
- To train and equip healthcare assistants and community care associates with necessary skills to care for our daycare clients.
- Carry out other responsibilities and duties as assigned from time to time, including coverage of other Staff Nurse in other Senior Care facilities if necessary.
Requirements
- Recognized qualification in Diploma/Degree in Nursing
- Registered with Singapore Nursing Board
- A minimum of 5 years’ experience in clinical area such as acute or community hospital.
- Good knowledge of the health care environment and service processes is important. Experience in community care provision is an advantage.
- A person with integrity, highly motivated and able to maintain good interpersonal relationships, in collaboration with various partners in the healthcare system.

Working Hours
- 4 days / 34 hours per week (8.5 hours per day)
