About The Role
The Community Intervention Team provides therapy, counselling, assessment, psychoeducation support, and case management for clients with dementia and mental health issues, including their caregivers. The assessment enables the team to create personalised care plans for clients or connect them with more fitting services. Caregivers will receive appropriate assessment and intervention, including support, training, psychoeducation and support groups.
COMIT Team works with clients aged 18 years and above.
Reporting to the Supervisor, HOD works closely with the team to serve clients and caregivers.
· Carry a caseload of clients under Casework and Counselling.
· Provide comprehensive mental health assessment, case management and psychosocial interventions.
· Monitor the progress of clients and work out aftercare plans for clients.
· Conduct group work for clients.
· Conduct community work.
· Plan and organise meaningful outreach and public education on mental health issues
· Collaborate with other agencies/organisations in planning and implementation of programmes
· Monitor trends and issues relating to mental health services and needs of clients and the community
· Network withstakeholders.
· Upkeep proper documentation and other administrative duties.
· Plan, implement and monitor the outreach, care process, referral and follow-up flow for the CREST service
· Document and evaluate programme progress
· Provide basic mental health awareness and information on managing the mental illness and coping strategies
· Provide basic emotional support to clients at-risk of mental health
· Compile and analyse collected data and submit reports as specified
· Other work projects assigned by supervisor/management
Technical Competencies:
- Degree in social work, counselling or any related allied health professional
- IT literate
Core Competencies:
- Good communication and social skills to work with different people
- Pleasant personality
- Team player, yet independent to carry out most functions
- Strong interest in empowering and preserving the dignity of people at-risk or with mental health conditions including dementia
Preferred achievements / characteristics:
- Able to speak dialects will be an advantage
