Job Description
1. Planning and Organising
- Regular liaisons with clients and families to follow up on pending assistance – Medifund, Household Means Testing (Subvention) and other funding schemes, etc.
2. Information and Referrals
- Provide up-to-date information about available resources, services and entitlements to residents and their families
- Assist residents and families in understanding their rights and responsibilities within the nursing home environment
- Application of AIC Long Term Schemes (Eldershield, PGDAS, IDAPE, Careshield)
3. Documentation and Reporting
- Maintain accurate and confidential records of assessments, care plans, documentations, interventions, and progress
- Maintain the inventory list of documentations and financial assistance
- Provide monthly report for Management
- Facilitates internal and external Audits
4. Collaboration and Teamwork
- Work with the team to provide holistic care for residents
- Participate in interdisciplinary team meetings to discuss resident progress and adjust care plans as necessary
5. Compliance and Ethical Considerations
- Adhere to ethical guidelines and professional standards set by relevant regulatory bodies
- Comply with organisational policies and procedures related to resident care and confidentiality
Requirements
- GCE “O” or “A” levels with 2 years of relevant healthcare industry experience
- Possess empathy and patience dealing with residents, next-of-kins and staff alike
- Demonstrate initiative to identify, plan, develop and implement resourcefulness to find solutions to missing gaps in work processes and mutually benefits stakeholders.
- Good communications and people engagement skills
- Good organisation ability
- Able to form networks and strong linkages with stakeholders
