Duties
- Identify and define patient-specific medical problems, recommend, implement and manage patient pharmacotherapy, review, counsel, dispense and/or supply medications as needed, provide medicine reconciliation, and ongoing monitoring and adjustment of patient's drug therapy, resolve and/or prevent patient's drug-related problems in accordance with departmental policies and procedures, work and/or consult with other healthcare providers to improve patient outcomes.
- Promote and monitor for adherence to prescribing guidelines or drug use protocols.
- Provide professional advice and evidence-based drug information to patients and the healthcare team.
- Work with patients, caregivers and other healthcare providers to ensure safe, costeffective, timely distribution and quality use of medication that will best meet the patients’ needs, achieve desired patient outcomes and assure continuity of services.
- Provide general clinical pharmacy services in assigned patient care area/service.
- Participate and collaborate within an interdisciplinary healthcare team to ensure optimization of medication therapy and outcomes for patients.
- Implement and review policies, practices and regulations established by the pharmacy department, various management committees and outside regulatory agencies e.g. MOH pharmacy practice standards, ENHS.
- Oversee purchasing operational issues and ensure the smooth running of the section – order processing, exemption drug application, billing, stock exchange/return, drug recall, follow-up, etc.
- Identify measures and work to achieve cost-saving through bulk purchase and negotiation with suppliers.
- In-charge of controlled drugs in the pharmacy – manage inventory, distribution maintain proper records, supply to inpatient wards, home care and day care.
- Ensure twice yearly medicine management audit is conducted
- Participate in Quality Improvement
- Undertake to keep oneself well-informed of the updated industry guidelines and develop new skills that are necessary for optimal functioning in the job.
- Participate in educational programmes and provide training to other healthcare professionals and public where require
- Participate in and contribute to continuing professional development.
Research - Initiate and/or participate in clinical research independently or in collaboration with other health care discipline
- Maintain high standards of housekeeping and workplace organisation at all times.
- Attends meetings as and when required.
- Participates in projects and any other duties as and when assigned
Requirements
- Minimally Degree in Pharmacy
- Prior experience in similar capacity is preferred
