Job Description:
- Conduct occupational/physio therapy specific assessment for clients during admission, periodic care plan reviews and discharge
- Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes
- Formulate person centered and SMART goals in collaboration with clients and relevant stakeholders
- Design plans and conduct occupational therapy interventions for clients in a person centered manner, that is also evidence based
- Document assessment results, therapy plans, interventions, goals and outcomes measures according to guidelines
- Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders
- Ensure proper use of equipment including technological-assisted rehabilitation equipment, train clients in the use of assistive device
- Recommend appropriate assistive device and equipment as needed
- Formulate discharge and continuity of care plans
- Facilitate community integration through collaboration with other community partners as required
- Participate in case discussion with the care team
- Support in the training and supervision of Therapy Assistants
