JOB SUMMARY
The Social Work Assistant's primary role will be to assist the rehabilitation counsellors in the areas of Medifund Financial Assistance, Case Management support, and/or Chaperone services to medical facilities. If necessary, the SWA will help clients access community resources such as housing assistance, healthcare services, employment support, and educational programs.
KEY DUTIES AND RESPONSIBILITIES
- Medifund: Assist the counsellors in administrative matters, i.e. compilation of necessary documents and signature procurement.
- Client support: Provide direct support for up to 15 clients. This may involve conducting pre-screening, intake interviews, assessing clients' needs and concerns, and assisting the rehab counsellors in the development and implementation of service plans.
- Community re-integration: Assist clients with applications for benefits or resources, connecting clients with community support networks, and ensuring clients receive appropriate care and support during their transition planning.
- Crisis Intervention: In times of crisis or emergency situations, assist the rehab counsellors to provide immediate support to their clients and/or families. The SWA may assist with assessing the situation, offerring emotional support, and connecting clients with emergency services as needed.
- Group facilitation: Assist in facilitating support groups or community programs within the centre. This includes helping to plan and organize group activities, ensuring a safe and inclusive environment, and providing guidance and support to participants.
- Documentation and reporting: Responsible for maintaining detailed and accurate records of client interactions, progress, and services provided. May also assist in preparing reports for supervisors or funding agencies.
- Collaboration: Collaborate closely with our networking partners, other professionals, and community organizations to ensure holistic and coordinated care for clients.
- Be up to date on the latest research and maintain a comprehensive knowledge of available resources and provide appropriate referrals if needed.
- Ethical and legal compliance: Adhere to professional and ethical standards, as well as legal and regulatory requirements. Required to maintain confidentiality, respect clients' rights and privacy, and report any concerns or issues to his/her reporting officer.
REQUIREMENTS
- GCE O level or Diploma qualification with at least 2 years of experience in the social service sector
- Passionate to work with persons with mental health issues
- Good interpersonal and communcation skills
- Able to speak local languages
- Proficient in Microsoft Office applications
- Strong organizational skills
